Starving Dog in need of Food and Medication

This Poor thing was found out in the woods behind my house. She is nothing but skin and bone and needs food, and a trip to the vet. However We can not do this for her alone. I am asking for the help of animal lovers everywhere. We want to keep her but can not unless she has had all of her shots and is checked from snout to paws. Please help us save this beautiful puppy.
